Most investigations identify what happened. Fewer identify why it happened. The best investigators move beyond rule violations and human error to uncover the organizational, supervisory, engineering, and cultural factors that allow incidents to occur. Using real-world investigations from construction and general industry, this presentation demonstrates practical techniques for asking better questions, identifying weak barriers, and developing corrective actions that prevent recurrence rather than simply assigning blame.
